Sarah Moss

Sarah joined ACM with over 20 years’ experience as an Occupational Therapist. Her Clinical Lead for Adult Case Management role at ACM is to provide bespoke case management input for Adult with brain injuries and other complex disabilities who require care, support and rehabilitation. She has particular experience of working with clients who have dual diagnosis of brain injury and mental health conditions. She works with clients both pre and post settlement of personal injury and medical negligence awards.

She has a varied case load of clients with cerebral palsy, brain injury, and complex orthopaedic injuries. Sarah is involved in all aspects of the case management role, and is an experienced report writer, preparing immediate needs assessment reports and costed case management reports. She has extensive experience in the coordination of services and managing professional multidisciplinary teams. She is passionate about sourcing the right therapists to work with her clients on their rehabilitation journeys.

Alongside her case management role, Sarah also works as a Neurological Occupational Therapist, within ACM’s Community Rehabilitation Service. She has particular expertise in cognitive rehabilitation approaches for clients to use in their everyday activities. This may include using memory strategies to enable a client to cook a meal, or manage other household tasks and improve their planning skills. Her work includes the provision of bespoke training sessions to care teams to assist them in developing and applying skills to support their client to live as independently as possible. She is experienced in supporting clients with work preparation skills, setting up work placements and monitoring these to ensure the placements meet the needs of the client. Sarah has completed Sensory Integration Level 1 module foundations of neuroscience which enables her to use sensory techniques to support clients with sensory needs.
