Dafydd Jones

Dafydd is an experienced case manager who specialises in working with Adult who have complex physical disabilities, such as brain injury, spinal injury, amputations, life-limiting conditions and those requiring palliative care. He ,also has experience working with individuals with learning disabilities and Adult and older persons with mental health issues or mental illnesses .He has also worked with individuals who have neurological disorders and life-limiting conditions. In his role, Dafydd manages a diverse team of health and social care professionals, overseeing complex assessments, care planning, and service coordination.
